CONGRATULATIONS ON YOUR SELECTION TO ONE OF OUR HEALTH PROGRAMS

You have been directed to this page based on your "SELECTION and ACCEPTANCE"

into one of FTCC's HEALTH PROGRAMS.

This page contains the information you will need to ensure a smooth transition into your program of study.

The items below relate to ALL of the Health Programs.

     1) PRE-ENTRANCE HEALTH FORM - ALL Students entering a Health program MUST complete and turn in

         a Pre-Entrance Health Form. 

         *Failure to meet all requirements listed on this form and obtain appropriate signatures from the

         medical facility may result in the student not being enrolled into the program. All immunizations

         listed on the form are required. It is preferred that completed forms be placed in the Health Records

         Drop Box on the 1st floor of the Health Technology Center (HTC) building, which is located on the

         wall just across from the elevator.

     2) ESTIMATED COST SHEETS FOR EACH PROGRAM may be viewed by CLICKING ON YOUR PROGRAM

         in the menu to the left.     

     3) UNIFORMS, SUPPLIES, AND EQUIPMENT:  Each program will have a different requirement.  Click

         on your program to the left to see their list of required items.

     4) CRIMINAL BACKGROUND CHECKS.  Obtaining the required background check is a student

          responsibility.  Your advisor will inform you of the process and time line required to obtain

          the required check.

******Criminal background checks are required for several Health Programs.  Students may be prohibited from clinical education at specific clinical affiliates based on background check results.  This could prevent a student from progressing in their respective health program.******
